In June 2008 the member agencies of the Central Highlands Primary Care Partnership identified the future vision for the CHPCP - 

        'Working together we build individual & community health and wellbeing'


It was agreed that through Service Design, Service Coordination, Health Promotion & Workforce Development  we will:

  • Improve Service Flexibility
  • Build community skills
  • Support participation in care
  • Provide affordable and accessible access to services
  • Provide access to information
  • Drive organisational cultural change


The member agencies also reviewed and refined the priority areas for the CHPCP.
