Diabetes Steering Group
The Diabetes Steering Group has been working toward creating an information brochure to be used as a service information guide for type 1 and type 2 diebetics in the Central Highlands Region. This brochure is planned for completion and release in early 2009.
The group have completed a number of tools to assist with referral and treatment of patients with diabetes, this includes a referral guide, triage tool and Diabetes Nurse Educator contact information.
Referral Guide - The guide is for use by General Practioners and other health professionals to assist with referral to a Diabetes Nurse Educator. This guide has been developed through the partnership work of Ballarat District Nursing and Healthcare, Ballarat Community Health Centre, Ballarat Health Services and St John of God Healthcare.

Triage Tool - The Diabetes Nurse Educator Intate Priority Tool can also be used as a screening tool for patients needing advice or support between appointments and a guide for how to proceeded with a client. Please note this tool is designed for use by clinical staff only and must be used in conjunction with the Guidelines: Diabetes Nurse Educator Priority Tool.

Diabetes Nurse Educator Contact Guide - This is a quick referance guide for use by General Practioners and other health professionals, listing contact details, services offered and referral methods for the Diabetes Nurse Educators in the Ballarat Region.

New Diabetes Resource Guide - The guide provides a wealth of practical information for people with diabetes, information on what services the different health professionals are able to provide as well as a list of services currently available across the Central Highlands region.
