Connectingcare Online Service Directory and e-referral System

Connectingcare is an online service directory and e-referral system used by 19 primary care partnerships across Victoria.  Connectingcare provides information on over 6000 services across Victoria. 

The e-referral system enables client information to be sent and received by encrypted email using existing Internet-based technology to create a secure platform for referral without the cost of additional software or equipment.  

Regular training sessions are held on E-Referral and Information maintenance

Agencies are able to list their service information free of charge on Connectingcare with the proviso that they will update the information regularly.
